1. Course Information

The 8-Hour Phlebotomy Seminar is an educational course designed to provide hands-on training and preparation for phlebotomy certification exams. This course is not affiliated with the National Healthcareer Association (NHA) and does not provide certification.

Course completion does not guarantee certification or employment. Students must separately register for and pass official certification exams through authorized testing centers.

2. Registration and Payment

Registration is confirmed upon submission of this form. Payment of $500 (or listed course fee) is collected on the day of the seminar in cash or via Cash App.

Failure to attend or pay on the seminar date will result in forfeiture of your registration spot.

3. Provider Rights and Course Management

Right to Cancel or Reschedule: We reserve the absolute right to cancel, reschedule, or modify any seminar for any reason without penalty, including but not limited to:

  • Low enrollment or insufficient registrations
  • Instructor illness or unavailability
  • Facility issues, closures, or venue changes
  • Weather conditions or natural disasters
  • Equipment malfunctions or supply shortages
  • Health and safety concerns
  • Government regulations or public health orders
  • Force majeure events beyond our reasonable control
  • Operational or business considerations

Course Modifications: We may modify course content, duration, materials, or methods of instruction without prior notice to ensure educational quality and safety standards.

Venue Changes: We reserve the right to change seminar locations and will provide alternative venue information with as much advance notice as possible.

4. Attendance Requirements

  • Students must arrive on time and attend the full 8-hour session
  • Valid photo identification is required for entry
  • Students must come prepared with appropriate attire for hands-on practice
  • Late arrivals may not be admitted and will forfeit their registration

5. Health and Safety

Students participate in hands-on training at their own risk. By registering, you acknowledge that:

  • You are in good health and able to participate in physical training activities
  • You understand the risks associated with phlebotomy training
  • You release the course provider from liability for any injuries sustained during training

6. Conduct and Dismissal

Students are expected to maintain professional behavior throughout the course. Disruptive behavior, failure to follow safety protocols, or inappropriate conduct may result in immediate dismissal without refund.

7. Limitation of Liability

The course provider's liability is limited to the amount paid for the course. We are not responsible for any indirect, incidental, or consequential damages arising from course participation.

8. Changes to Terms

We reserve the right to modify these terms at any time. Changes will be posted on our website and apply to future registrations.
